Discrimination of hip fracture event using a 3D reconstruction method from 2D DXA - A case control study

  • Osteoporosis is a skeletal disease characterized by a decrease in bone mass and density leading to enhanced bone fragility and a consequent increase in fracture risk. This pathology affects more than 75 million people in Europe, Japan and the USA, and was responsible of 1.3¿1.7 million of hip fractures worldwide in 1990. The number of hip fractured cases is expected to double by 2025 due to increasing in life expectancy. Dual¿energy X¿ray Absorptiometry (DXA) images are used in clinical routine to measure the areal Bone Mineral Density (aBMD), as aBMD is correlated with Bone Strength and therefore with fracture risk. This diagnosis is however limited by its bidimensionality, not taking into account the 3D distribution of BMD. To overcome this limitation, methods have been developed to model in 3D the bony shape and BMD distribution from 2D DXA images and to compute, from those 3D models, a volumetric BMD (vBMD) quantifying the femoral strength. The objective of the present work is to evaluate how such a 3D vBMD computed from 2D DXA images can discriminate between patients with and without hip fracture. A comparison with clinical routine 2D aBMD extracted directly from a DXA image of the femur is made in order to see the improvement of the 3D method under study.
